If you need to enroll with a health plan and PCP for the first time, you can enroll three ways:

PHONE: Call Illinois Client Enrollment Services at 1-877-912-8880 (TTY: 1-866-565-8576). The call is free. Illinois Client Enrollment Services will help you pick a PCP.

MAIL: Fill out the enrollment form you received in the mail. Send the form back to Illinois Client Enrollment Services in the envelope provided.
